This form should be submitted when you have completed all the tasks in the volunteer orientation course.
When feedback is requested we encourage you to provide positive and negative feedback to help us improve the course.
On receipt of this form we will assess your contributions throughout the course and get back to you with either our recommendations or your graduation details.
On graduation you will be awarded a CPD certificate and your Yellow Belt badge. We will then discuss your desired future volunteer role.
