Documentation is important in all areas of healthcare and rehabilitation. High-quality documentation ensures consistent patient care and enhances communication among the multidisciplinary team. It also supports research and can be used in medico-legal scenarios. Proper documentation is essential for rehabilitation professionals to monitor a patient’s medical history, track their progress, investigate issues and make informed decisions easily. All rehabilitation professionals should be trained and skilled in providing high-quality documentation for every patient interaction and discussion related to patient care. In this course, the key principles of documentation will be discussed.

At the end of this course you will be able to:
- justify the application of four principles underlying the creation of high-quality documentation in rehabilitation
- differentiate between the benefits of good documentation and the consequences of poor documentation
Target audience
This course is aimed at rehabilitation professionals, students and assistants including but not limited to Physiotherapists, Occupational Therapists, Speech and Language Therapists, Rehabilitation Doctors, Rehabilitation Nurses, Prosthetists, Orthotists, Psychologists, Audiologists, Dietetics, Social Workers. Community Health Workers, Nurses or Medical Doctors interested in this subject are also invited to participate.
