Introducing the PAI “Discharge Report” task

Last month we launched the “Tasks” feature of the Physiopedia AI Assistant (PAI) and have been exploring each task option in more detail. How PAI’s “Discharge Report” task works

The PAI Discharge Report task guides you through the process of efficiently creating comprehensive, professionally structured discharge documentation. To create a discharge report, you’ll start a new conversation with PAI and share a summary of a clinical scenario. You can share key details about initial presentation, treatment progression, outcome measures and status at discharge in a quick informal style through using the dictation feature of your chosen device.

When you request the Discharge Report task, PAI analyses the information you’ve provided in your conversation and organises it into a structured discharge summary. The report typically includes sections for demographics, referral information, initial assessment findings, treatment interventions, outcome measures, goals achieved, current status and recommendations for continued management or self-care.

What sets this PAI task apart is its interactive, educational approach to discharge documentation. Rather than simply generating a static report, PAI actively guides you through the process by prompting for specific information that might be missing, helping you understand what a comprehensive discharge report requires. As you work through the process together, you can request amendments, developing your awareness of how formatting, terminology and emphasis can be tailored to meet the specific needs of different audiences. Daniel uses the PAI Discharge Report task

It’s Wednesday afternoon and physiotherapy student Daniel has just completed an observed session with a patient who has been receiving treatment for chronic low back pain over the past eight weeks. As part of his placement learning, Daniel needs to produce a discharge report — an unfamiliar document type he hasn’t written independently before.

He opens the Plus app and starts a new conversation with PAI. Drawing from his clinical notes, Daniel describes the patient’s initial presentation, the eight-week treatment progression, key outcome measure improvements and current status at discharge. He then clicks the Tasks button and selects the Discharge Report task.

PAI works through the process with Daniel, prompting him to consider what information a referring general practitioner would need, and why each section of the report matters. When PAI asks about ongoing self-management strategies, Daniel realises he hadn’t initially thought to include this — a useful reminder of what comprehensive discharge planning involves.

PAI generates a structured draft, and Daniel reviews it carefully. He notices the clinical outcomes section doesn’t clearly highlight the patient’s functional improvements and, guided by PAI’s suggestions, works through how to frame these more meaningfully for a GP audience. By the end of the process, Daniel has a well-structured discharge report — and a much clearer understanding of what good discharge documentation looks like and why.

Accessing the Discharge Report task

To try this new PAI task, log in to your Plus account. On any Plus page, open PAI by clicking the Ask PAI? button that is always visible at the bottom right of the page. In the PAI panel, click the Tasks button. Scroll the Tasks list and click the “Request” button for the Discharge Report task.
