The Physiopedia Clinical Case Simulator is an interactive tool, powered by the Physiopedia AI Assistant (PAI), that generates simulated patient scenarios so you can practise your clinical reasoning. You make every clinical decision and PAI prompts you to reason through the case rather than handing you the answers.
The Physiopedia Clinical Case Simulator is an AI-driven practice tool, run by the Physiopedia AI Assistant (PAI), that generates realistic patient scenarios for clinical reasoning practice, and it is available exclusively on Physiopedia Plus (Plus). It introduces a simulated patient and guides you through a full clinical encounter, and because every patient is simulated for practice, no real patient data is involved at any stage. You decide what to ask and what to examine, and the simulator responds as the case unfolds. It releases findings only when you request them, flags when you have skipped something important, and holds the diagnosis until your reasoning gets there.
The simulator suits anyone who needs case practice without a real caseload. Physiotherapy students can prepare for placements or exams, new graduates can build confidence in areas they rarely see, experienced clinicians can practise outside their usual caseload, and educators and clinical supervisors can find ready-made cases for teaching and small-group work.
To begin, you pick the clinical specialty and the difficulty, from beginner through intermediate to advanced, and the case is built around your choice. Every case then runs through three phases: assessment, reasoning and management. In assessment, you work through the subjective history, objective findings and outcome measures by choosing what to ask, with nothing revealed until you request it. In reasoning you build a concise problem representation, form a working hypothesis and reflect on the biases that can lead to diagnostic error. In management you set out what you would do next, weighing referral decisions, safety considerations and what good practice looks like. PAI then closes the case with personalised feedback on your clinical strengths, your core case competencies and clear areas to develop. When presenting the feedback PAI gives you a Pass or Fail for the case, and as you build your cases, you’ll see each one appear on your clinical case simulator dashboard, with a button to download certificates for those that you pass.
The simulator behaves very differently from a general-purpose AI chatbot. You can discuss a case with either, but where a chatbot tends to summarise and answer immediately, the simulator keeps you in charge of the reasoning throughout.
| The case simulator | A general-purpose AI chatbot | |
|---|---|---|
| Who runs the case | You do, and findings are released only when you request them | It summarises and answers immediately |
| Diagnosis | Withheld until your reasoning gets there | Often given in the first reply |
| Structure | Fixed assessment, reasoning and management sequence | Unstructured |
| Bias awareness | Explicitly prompted | Rarely raised |
| Feedback | Strengths, core case competencies and development areas, downloadable certificate | Not built in |
| Grounding | Built on Physiopedia’s quality-assured knowledge base with expert oversight | General web training data |
| Data privacy | Designed to prevent the storage of personally identifiable information | Inputs retained, without the same safeguards |

A few things people often ask before they begin. You need a Full or Pro Plus subscription (although new Trial users can use it for the first seven days of membership). It is case practice and an educational tool rather than a substitute for supervised clinical experience. And a typical case takes around 20 to 40 minutes, though you can stop at any point and still get feedback. You will find more on the Physiopedia Plus Help page.
