PAI, the Physiopedia AI Assistant, was built not to follow the AI trend, but to ensure physiotherapists had access to an AI tool genuinely designed around their clinical needs. Here, Physiopedia’s CTO Tony Lowe explains the thinking behind PAI and what makes it different from every other AI tool on the market.
A few years ago, we watched artificial intelligence (AI) move from a niche technology story into something that was genuinely reshaping how people worked, learned and made decisions across almost every profession. Our response was clear: physiotherapists deserved to be part of this, but they deserved something built for them, tailored to their clinical needs, grounded in their evidence base and designed around how they actually work. Not a generic tool handed down from the tech world and left to the profession to figure out.
That belief is what drove us to build PAI, the Physiopedia AI Assistant. The response has been extraordinary. In 2025 alone, PAI was used by over 10,000 healthcare professionals across 159 countries, facilitating more than 54,000 conversations. And 2026 looks set to break that – since January over 9,100 physiotherapists have already continued that momentum. That kind of global adoption points to one thing: the profession was ready for this. It just needed the right tool.
The question we get asked most is: “Why not just use ChatGPT?” Our answer is that for general tasks, generic AI tools are fine. For clinical practice, they are not, and the distinction matters. General AI tools are trained on the entire internet: the good, the bad, the outdated and the dangerously oversimplified, all blended together with no clinical filter and no professional accountability. When a physiotherapist asks a generic AI a clinical question, they are getting an answer built from whatever the internet happens to contain on that topic, with no traceable source and no quality assurance.
PAI is different because of what it is built on. Over 15 years, Physiopedia’s global community of physiotherapy professionals has created and maintained 5,843 pages of rigorously reviewed, evidence-based content. Add to that 833 internationally accredited Physiopedia Plus courses and thousands of expert videos from leading clinicians worldwide. That is the knowledge base PAI draws from exclusively. Every answer comes with traceable sources so clinicians can follow the evidence, verify the information and trust the response. Critically, PAI never shares or uses your information. Total data protection is built into the architecture from the ground up.
What PAI can do goes well beyond answering questions. Since the launch of PAI Tasks in 2025, physiotherapists have access to ten purpose-built practice education support tools designed to reduce the administrative burden that eats into precious patient time. From guiding you through the creation of detailed SOAP notes that reflect your clinical reasoning, to producing personalised patient education guides, these tools are designed to support your thinking and document your decisions accurately. They take care of the structured work so that clinicians can focus on what matters most.
We are also proud to have become the first physiotherapy online learning platform to integrate AI-supported learning activities directly into our courses. These interactive sessions guide learners through realistic clinical scenarios using Socratic dialogue and complex case-based reasoning, bridging the gap between theoretical knowledge and practical application. PAI knowledge checks, available in every Plus course, go beyond standard multiple choice to create dynamic, adaptive evaluations with immediate personalised feedback.
One user described the experience perfectly: “It feels like you’re genuinely talking with someone who guides you to explore and deepen your thinking in ways you might not have considered on your own.” That is exactly what we set out to build.
