The referral letter challenge
For physiotherapists, creating effective referral letters is a critical but often time-consuming aspect of practice. A well-crafted referral letter needs to clearly communicate patient history, assessment findings, interventions attempted and specific reasons for referral – all while maintaining professional tone and adhering to clinical communication standards.
Many clinicians experience spending significant time drafting, editing and refining referral letters, particularly when complex cases require specialist input. This administrative burden can take away from valuable patient treatment time and contribute to end-of-day documentation stress. The PAI “Referral Letter” task aims to support physiotherapists in developing their ability to efficiently create comprehensive, professionally formatted referral letters.
Developing confident clinical communication
How PAI’s “Referral Letter” task works
The PAI Referral Letter task leverages your clinical conversations to guide you in developing tailored professional correspondence. You can request this task during or after discussing a clinical scenario with PAI, and the assistant will work with you to identify the key information and structure it into an effective referral letter format.
When the task is requested, PAI works with you to review the clinical information shared during your conversation and guides you in organising it into a comprehensive referral letter with appropriate sections: patient information, clinical history, assessment findings, interventions, outcomes and specific reasons for referral. The resulting letter maintains professional language and focuses on communicating the most clinically relevant details.
Most importantly, the generated referral letter is not a static document – it’s the starting point for refinement. After reviewing the initial draft, you can ask PAI to modify specific sections, emphasise particular clinical findings or condense certain aspects to better serve your referral needs. This iterative process ensures the final letter precisely communicates your clinical reasoning and referral requirements.
Emma’s referral letter
It’s Thursday afternoon, and Emma has been treating a patient with persistent shoulder pain that hasn’t responded to the expected timeline of conservative management. She recognises that an orthopaedic consultation may be beneficial but dreads the thirty plus minutes it typically takes her to craft a comprehensive referral letter.
Instead, Emma opens the Plus app on her tablet, selects the PAI tab and requests the Referral Letter task. PAI guides her through the key elements of an effective referral letter, asking related questions to help her structure her thinking. Reviewing the letter, Emma notices it includes more detail about early treatment phases than necessary, but lacks emphasis on the recent plateau in progress.
She dictates into the ongoing PAI conversation “Please revise this letter to focus more on the recent plateau in shoulder range and pain levels, and reduce the details about initial treatment phases.” Working through the revision with PAI, Emma develops a letter that better emphasises her clinical reasoning and specific concerns. She makes a few final personal edits, satisfied that the letter clearly reflects her professional judgement.
Accessing the referral letter task
To try this new PAI task, log in to your existing Plus account. On any Plus page, open PAI by clicking the Ask PAI? button that is always visible at the bottom right of the page. In the PAI panel, click the Tasks button. Scroll the Tasks list and click the “Request” button for the referral letter task.

- After requesting the task follow the question and answer structure to provide the relevant details.
- Review the initial draft thoroughly and ask for specific revisions where needed
- Consider the receiving clinician’s perspective, what information is most relevant to them?
- Don’t be shy, ask PAI to amend your initial letter! Remember that iterative refinement often produces the best results.
