What physiotherapists need to understand to get the most out of AI

Artificial intelligence (AI) is already everywhere in healthcare, including physiotherapy. From evidence-based treatment recommendations to clinical decision-making support, these tools can enhance your practice when used correctly. The key lies in understanding how to communicate effectively with AI systems through strategic prompting and recognising how this differs from traditional search methods.

1. Know what an AI prompt is

An AI prompt is a question or instruction that guides how an AI system responds. The AI uses your prompt to understand your request, then searches its knowledge base to generate a response. 2. Understand the difference between AI prompts vs. internet searches

Internet searches find what already exists. When you use Google or search within Physiopedia, you’re locating existing information. Think of it like asking a librarian to find a specific book.

Best practice: Use simple, specific terms with Boolean operators: “Knee AND arthroplasty AND exercises”

AI prompts create something new. AI generates content based on its training data, like asking a writer to compose a new story. There’s no one “correct” answer stored somewhere; each response is newly generated.

Best practice: Use detailed, descriptive prompts: “Create a comprehensive home exercise programme for a patient 4–6 weeks post knee arthroplasty, including progression criteria and precautions”

3. Be specific – AI is only as useful as your prompt

Unlike search engines, AI systems thrive on context and specificity. The more details you provide, the better the response.

For example, clearly identify the medical condition (specify “wrist extensor tendinopathy” rather than “wrist pain”), include your practice setting, patient demographics and treatment goals. The more detailed your prompt, the more useful the response.

4. Structure your prompt using the Role-Goal-Instruction (RGI) method

The RGI framework creates effective AI prompts by focusing on professional context, targeted outcomes and specific tasks.

Role: Tell the AI how you want it to behave and the perspective you want it to provide
Goal: Explain what you want to achieve
Instruction: Give specific directions and/or task description

Example:

Role: You are an experienced physiotherapist specialising in sports injuries
Goal: To create a rehabilitation programme for a high school athlete recovering from an ACL tear
Instruction: Design a 12-week rehabilitation plan, including exercises, progression milestones, and precautions. Include both clinic-based and home exercises suitable for a 16-year-old soccer player.

Tip: when using PAI you do not need to include the “Role” element in your prompt as this is built in!

5. Engage in conversation to improve your output

Unlike search engines, AI supports and thrives on conversations. Your first prompt is just the beginning. After receiving an initial response, you can:

  • Ask for clarification: “Can you explain the rationale behind exercise #3 in more detail?”
  • Request modifications: “Adapt this programme for a patient with limited mobility who uses a wheelchair”
  • Seek alternatives: “What would you change if the patient also had diabetes?”
  • Add constraints: “Modify this to work with equipment available in a home setting only”

6. Know the limitations

AI can make mistakes and produce inconsistent responses, so don’t expect 100% accuracy or identical answers to the same question on different days. You’re jointly responsible for outcomes: weak prompts generate weak responses. If the answer isn’t meeting your needs, refine your prompt with better context and instruction. And just as you would with human interactions, always double-check important information and use your clinical judgment before applying anything to practice.
